  • Live in Oregon
  • Are pregnant, postpartum (up to 6 months after delivery), or breastfeeding or an infant or child under 5 years of age.
  • Have a nutritional need as determined by our professional nutritionist
  • Have a household include that is less than or equal to the guidelines below:
